considered therapy. Consultations over the Internet are not the same as
face-to-face therapy. By using this website, I agree to the following:
1. All consultations shall be deemed taking place in California where Dr.
Armstrong is licensed and where I am contacting him;
2. I realize that communications over the Internet cannot be completely
secure, and I agree to waive any confidentiality issues which may arise
during my exchanges with Dr. Armstrong;
3. I understand that Dr. Armstrong is under an ethical obligation to
disclose to the authorities any child or elder abuse, any serious or
irreversible harm to myself, and to warn anyone who might be in danger if a
threat is made against them;
4. I understand that it is my responsibility to seek psychological or
medical help if it is needed;
5. I understand that the purpose of this exchange is for me to ask Dr.
Armstrong questions, which he will answer to the best of his ability, but
owing to the nature of this exchange and the limitations inherent in
communications through this media, Dr. Armstrong may not be able to engage
in the same type of in-depth evaluation as he would during a face-to-face
meeting. I, therefore, agree to hold Dr. Armstrong harmless against any and
all liability arising from my exchanges with him.
